We collect personal information about you, including health and sensitive information, when you access, view or use the content, material or services on or through this website, in order to respond to your enquiries, conduct your medical specialist or patient onboarding with the medical specialist, process your referral and to provide you with other services that we provide.

We may disclose your personal information to third parties including those involved in your health care, such as our medical specialists personally involved with your relevant care. We may also disclose your personal information to our related entities and third parties that help us deliver our services (including our patient onboarding administrator, our marketing contractor, IT systems and cloud storage providers, email marketing providers and payment providers) or as required by law. If you do not provide this information, we may not be able to provide all of our services to you.

By providing your personal information to us, you consent to the collection, use, storage and disclosure of that information as described in these Terms.

We may also disclose your personal information to recipients that are located outside of Australia, including to payment providers located in the United States.

Our Privacy Policy explains:

(i) how we store and use, and how you may request to access, correct and withdraw your personal information; (ii) how you can lodge a complaint regarding the handling of your personal information; and (iii) how we will handle any complaint. 2. Additional terms and conditions

For certain services or activities relating to Creston Health Concierge additional terms and conditions may apply. For example, your offering of medical services to our patients is subject to our Medical Specialist Service Provider Agreement. Also, if we connect you with a medical specialist, you need to agree to the relevant terms and conditions of that medical specialist. In case of any inconsistency between such terms and conditions and these Terms, those terms and conditions will prevail.

3. Accuracy, completeness and timeliness of information

The information on our website is provided for general informational purposes only, is not comprehensive, and is intended to provide a summary of the subject matter covered. The services provided by Creston Health Concierge are of a general nature only and are not intended to be a substitute for professional medical advice, diagnosis, or treatment. By using our services that connect you with our medical specialists, you acknowledge and agree that Creston Health Concierge:

  1. does not provide medical advice;

  2. acts solely as a platform to facilitate connections between patients and independent medical specialists; and

  3. bears no responsibility for any medical advice, diagnosis, or treatment provided by medical specialists as a result of your registering as a patient on our platform.

You should always seek the advice of qualified healthcare professionals regarding any medical condition or treatment.

9. Right of removal

We reserve the right to remove, at any time, any Content, including, without limitation, any and all images, photographs, written information or other material on or from the website.

10. Unacceptable activity

You must not do any act that we would deem to be inappropriate, is unlawful or is prohibited by any laws applicable to our website, including but not limited to:

  • any act that would constitute a breach of either the privacy (including uploading private or personal information without an individual's consent) or any other of the legal rights of individuals;
